Regular Class Sign Up Agreement

I agree to take lessons as outlined on the sign up webpage with Mountain Kim Martial Arts the length of the allocated for the lessons is prorated at a minimum of one (1) lesson per week. I further understand that failure to complete the lesson does not relieve me of my obligation to pay the tuition in full and, therefore, this agreement is non-cancelable and non refundable. Lessons are not conducted on national holidays. No credit will be given for missed classes.

I understand that in consideration for services to be rendered by Mountain Kim Martial Arts, I agree to make payments to Mountain Kim Martial Arts, until the tuition for the term of the agreement is paid in full.

I am paying a down pay, and upon execution of this agreement, it will be applied to the said term tuition fee, and will begin making the aforesaid payments, and will continue to make the said monthly payments on the agreed day. I understand that this agreement is to be valid for the term specified and beginning on the date of execution of this agreement. If the account becomes delinquent, the customer agrees to pay attorney and/or collection fees. A service charge of $35 will be applied to all checks returned for insufficient funds.

I understand and agree that under the terms of this agreement, Mountain Kim Martial Arts will provide competent instructors and suitable facilities for teaching lessons. Qualified personnel trained in the skills of martial arts supervise all classes, practice sessions, examinations and contests. Official belt recognition and certification will be issued upon the successful completion of examinations (at an additional charge).

I understand that strict observation by me of the rules and regulations relative to my training, are the same as set out by Mountain Kim Martial Arts, will largely eliminate the possibility of accident or injury, but I hereby waive any claim of personal injury or damages against Mountain Kim Martial Arts, or any of its principles, instructors, agents, or employees in any case resulting from the subject activity.
